Do I need international health insurance in Thailand?

Thailand has no single rule requiring every foreign resident to be insured, but several long-stay visas do. The Non-Immigrant O-A requires cover of at least USD 100,000 (THB 3,000,000), and the LTR visa requires at least USD 50,000 or an accepted alternative. As foreigners usually pay for treatment themselves, most long-stay residents take out private or international cover.

Es hängt ab von

Your visa

The O-A and LTR visas set minimum cover amounts, the Non-O retirement rules vary by embassy, and the published DTV requirements do not list health insurance.

Whether you work for a Thai employer

Employees registered with Thai social security have a basic public scheme, which the LTR rules accept in place of private insurance.

Your age and health

Premiums and exclusions rise with age and pre-existing conditions, which matters because O-A cover must run for the whole stay.

Wer es braucht

Anyone moving to Thailand from Germany for longer than a holiday should plan for health cover. Retirees applying for the Non-Immigrant O-A must show insurance for the entire stay, and LTR applicants must hold a qualifying policy unless they use Thai social security or a large bank deposit instead. Remote workers on the DTV are not asked for insurance by the embassies, but they pay hospital bills themselves. Once you deregister (Abmeldung) and leave German statutory health insurance (gesetzliche Krankenversicherung), German cover generally no longer pays for treatment in Thailand, so an international or Thai private policy usually fills the gap.

Wie es funktioniert

  1. 1

    Check your visa's insurance rule

Confirm the current requirement with the Thai mission handling your application; for residents of Germany this is usually the Royal Thai Embassy in Berlin. Requirements differ between missions.

  2. 2

    Choose a sufficient sum insured

For an O-A visa the policy must cover at least USD 100,000 (THB 3,000,000) for the whole stay; for the LTR visa at least USD 50,000.

  3. 3

    Get the insurer's certificate

For O-A applications, embassies ask for the insurer to complete a 'Foreign Insurance Certificate' form rather than relying on a general policy schedule.

  4. 4

    Settle your German cover

Before deregistering, ask your Krankenkasse how membership ends and whether a dormant membership (Anwartschaftsversicherung) makes sense if you may return.

    before you deregister
  5. 5

    Keep the cover continuous

LTR conditions, including insurance, must be maintained for the life of the visa; renew on time and keep proof ready for immigration.

Häufige Fehler

  • Assuming a short-term travel policy meets the O-A requirement, which expects cover at the stated minimum for the whole stay.
  • Relying on German statutory health insurance after deregistering, even though it generally no longer pays for ongoing treatment in Thailand.
  • Choosing a policy whose annual limit is below the visa minimum, which can lead to a refused application.
  • Reading the DTV's lack of an insurance requirement as a sign that cover is unnecessary, when patients pay their own hospital bills.
  • Letting a policy lapse between renewals, which can mean new waiting periods or exclusions with the next insurer.

FAQ

Does the Destination Thailand Visa (DTV) require health insurance?

The DTV requirements published by Thai embassies ask for financial evidence of THB 500,000 but do not list health insurance. That does not mean treatment is free: foreigners generally pay private and public hospitals directly. Most DTV holders therefore arrange their own cover, either a Thai private policy or an international plan that also covers trips back to Germany.

Can I use Thai social security instead of private insurance?

For the Long-Term Resident (LTR) visa, yes: the BOI accepts current Thai social security cover as an alternative to a policy of at least USD 50,000. Social security is normally available only to people employed by a Thai employer, so it rarely helps retirees or remote workers. Its scope is also more limited than many private plans.

Is the old THB 40,000 / 400,000 rule still used?

Some missions still list it. The Royal Thai Embassy in Lima, for example, asks Non-O retirement applicants for outpatient cover of at least THB 40,000 and inpatient cover of at least THB 400,000, alongside a total of THB 3,000,000. For the O-A visa, embassies ask for at least USD 100,000 (THB 3,000,000). Always check your own embassy's list.

Do I need insurance on an LTR visa if I keep money in the bank?

The LTR rules accept at least USD 100,000 held in a bank account for 12 months or more instead of insurance. That satisfies the visa, but any medical bills then come out of your own funds. Many holders still buy a policy so that a serious illness does not erode the deposit they need to keep the visa.
